Unacceptable specimens:

Specimens will be rejected if clotted, haemolysed, low in volume, labelled improperly, wrong container and improperly stored or transported.
Other specimens may have a prolonged turnaround times because lack of necessary ancillary specimens or patient information.
You will be notified of rejected or problem specimens upon receipt.
To avoid specimen rejection, each specimen must be accompanied with a complete REQUEST FORM that provides:

  The patient full name, medical record number, date of birth and gender (If forename has not been established for a new-born baby, requisition form must contain surname).
  Specimen type, time/date of specimen collection and test desired.
  Clinical indications, history and therapy (for oncology).
  Referring physicians name and contact number.
  Three generations pedigree should be provided for all patients.
  Specimen must contain full name and medical record number.
  Specimen containor, transportation temperature and transport medium must be checked

Billing:

GenaTi's client will receive an itemized invoice/ statement, which will indicated the date of service patient name, Requisition Number, test name, and test charge. Payment terms are based on the contractual agreement.

Cancellation of Tests:

Request for cancellation must be via fax or email, and will be honored at no charge if received prior to test setup. Cancellation request after test setup cannot be honored. A report will be issued automatically and charged appropriately.

Additional Tests (Add-ons):

Requests for additional orders on specimens received by GenaTi must be confirmed via Fax or email. Call to check on sample requirements and availability to avoid any rejections.

Test Result call back and confidentiality:

Result will be faxed to a pre assigned fax number when requested by a phone call to GenaTi from the client. GenaTi endeavors to maintain the confidentiality of all patients information.

Unsatisfactory Analytical Result:

If GenaTi is unable to obtain a satisfactory analytical result, there will be no charge.

Test Turnaround Time:

Detailed turnaround time for each test will be available on our website. Repeated tests will take additional turn around time.

State and urgent orders:

All referred orders will be treated as routine and will be accommodated according to the laboratory working hours. STAT and urgent processing request must be approved by designated person before submitting the order and sending the specimens. Processing approved STAT or urgent orders will waiver any contractual discounts and will be charged by the full listed price of the order.

Unlisted test:

New procedures are developed throughout the year; therefore, some tests are not listed in our request form, booklet or website. For information about unlisted test, contact GenaTi general inquiry.

Supplies:

Special specimen collection containers and kits, sterile vials and request forms are supplied without charge.

Informed Consent Certification:

Submission of an order for any test constitutes the certification to GenaTi by the ordering physician that:

  The ordering physician has obtained the (Informed Consent) of the subject patient as required by applicable law with respect to each test order
  The ordering Physician has obtained from the subject patient authorization permitting GenaTi to report the results of each test ordered directly to the ordering physician.
